Immigration Physicals

Complete USCIS-required immigration physical examinations performed by a designated civil surgeon. We handle the Form I-693, required vaccinations, blood work, and TB screening — all in a welcoming, respectful environment.

What the Immigration Physical Includes

The immigration physical examination is a mandatory component of the United States green card application process. Performed by a USCIS-designated civil surgeon, this evaluation confirms that applicants meet the health-related requirements for lawful permanent residency.

The examination includes the following components:

  • Medical history review — discussion of past and current medical conditions, surgeries, hospitalizations, and mental health history
  • Physical examination — comprehensive head-to-toe evaluation including assessment of skin, eyes, ears, nose, throat, heart, lungs, abdomen, lymph nodes, extremities, and neurological function
  • Tuberculosis screening — either a TB skin test (TST/PPD) or an interferon-gamma release assay (IGRA) blood test; positive results require a follow-up chest X-ray
  • Blood tests — serologic testing for syphilis (RPR) and other conditions as required by USCIS
  • Vaccination review and administration — verification of vaccination history against USCIS requirements, with any missing vaccinations administered during the visit
  • Completion of Form I-693 — the designated civil surgeon completes and signs the official Report of Medical Examination and Vaccination Record

The completed Form I-693 is placed in a sealed envelope for you to submit with your adjustment of status application. The form is valid for two years from the date the civil surgeon signs it.

Required Vaccinations for Immigration

USCIS requires applicants to demonstrate proof of vaccination against specific diseases as part of the immigration physical. If you do not have documentation of previous vaccinations, or if your records are incomplete, our clinic can administer the required vaccines during your visit.

The current USCIS-required vaccination list includes:

  • Measles, Mumps, Rubella (MMR)
  • Tetanus, Diphtheria, Pertussis (Tdap)
  • Varicella (chickenpox)
  • Influenza (seasonal, if applicable during flu season)
  • COVID-19
  • Hepatitis B
  • Polio (IPV) — for applicants under 18
  • Rotavirus — for infants
  • Haemophilus influenzae type B (Hib) — for children
  • Pneumococcal — for specific age groups

Some vaccinations require a series of doses administered over weeks or months. Your civil surgeon will advise you on which vaccines can be completed during your visit and which may require a follow-up appointment. We work to minimize the number of visits needed while ensuring full compliance with USCIS requirements.

What to Bring to Your Appointment

Being prepared for your immigration physical helps ensure a smooth, efficient experience. Please bring the following documents to your appointment:

  • Valid government-issued photo identification (passport, state ID, or driver's license)
  • Any existing Form I-693 provided by your immigration attorney, if applicable
  • Complete vaccination records from your home country and any vaccinations received in the United States
  • Prior medical records, including any history of TB testing or treatment, mental health treatment, or substance use treatment
  • A list of current medications
  • Any documentation from previous immigration medical exams

Please note that the immigration physical is a self-pay service and is not covered by insurance. We provide transparent pricing that includes the examination, Form I-693 completion, standard blood work, and TB screening. Vaccinations are billed separately if administration is required. Payment is collected at the time of service. We accept cash, credit cards, and debit cards.

Turnaround time for the completed Form I-693 is typically within five to seven business days after all lab results are received. If a chest X-ray is required due to positive TB screening, this may extend the timeline slightly. We understand the time-sensitive nature of immigration applications and work diligently to return your documents as quickly as possible.

What is a designated civil surgeon?

A designated civil surgeon is a physician authorized by USCIS to perform immigration medical examinations and complete Form I-693. Only designated civil surgeons can conduct these exams — a regular doctor's physical does not satisfy immigration requirements. Hybrid Health Systems has a designated civil surgeon on staff.

How long is the Form I-693 valid?

The Form I-693 is valid for two years from the date the civil surgeon signs it. However, USCIS must receive the form no more than 60 days before you file your adjustment of status application, or at any time after filing. Your immigration attorney can advise on optimal timing.

What if I do not have my vaccination records?

If you cannot provide vaccination records, the civil surgeon may order blood tests (titers) to check for immunity to certain diseases. If titers show immunity, the vaccination requirement is satisfied. If immunity is not demonstrated, the vaccines will need to be administered.

Does insurance cover the immigration physical?

Immigration physicals are generally not covered by health insurance. This is a self-pay service with transparent, upfront pricing. Payment is collected at the time of service. We accept major credit cards, debit cards, and cash.

Can I complete the immigration physical in one visit?

In many cases, the examination, blood work, and vaccinations can be completed in a single visit. However, some vaccinations require a series of doses over time, and TB test results require a 48 to 72 hour read. Your civil surgeon will outline the expected timeline during your first appointment.

Do you offer immigration physicals in languages other than English?

We strive to accommodate patients who speak languages other than English. Please let us know your language needs when scheduling so we can arrange appropriate interpreter services or connect you with a bilingual staff member when available.
