The DOT physical examination covers the following areas:

  • Vision testing — minimum 20/40 acuity in each eye (with or without correction) and a 70-degree field of vision in both horizontal meridians
  • Hearing assessment — ability to perceive a forced whisper at five feet or audiometric testing equivalent
  • Blood pressure and pulse evaluation — readings must fall within acceptable ranges for certification
  • Urinalysis — screening for underlying conditions such as diabetes, kidney disease, or other metabolic concerns
  • Complete physical examination — musculoskeletal assessment, neurological evaluation, cardiovascular review, respiratory function, and abdominal examination
  • Medical history review — discussion of current medications, past surgeries, chronic conditions, and any prior DOT exam findings

Note that the DOT physical urinalysis is a health screening, not a drug test. Drug and alcohol testing is a separate process governed by different regulations.

Who Needs a DOT Physical

Federal law requires a valid DOT medical certificate for anyone who operates a commercial motor vehicle in interstate or intrastate commerce. This requirement applies broadly across several categories of drivers and vehicle operators.

Individuals who need a DOT physical include:

  • Commercial Driver's License (CDL) holders operating trucks, tractor-trailers, and other heavy vehicles
  • Bus drivers, including school bus and public transit operators
  • Hazardous materials (hazmat) transport drivers
  • Operators of vehicles with a gross vehicle weight rating (GVWR) over 10,001 pounds
  • Drivers transporting 16 or more passengers (including the driver)

DOT medical certificates are valid for up to 24 months, though certain health conditions may result in a shorter certification period. Drivers with well-managed conditions like controlled hypertension may receive a one-year certificate with annual recertification requirements.

How to Prepare for Your DOT Physical

Arriving prepared for your DOT physical helps the process go smoothly and reduces the chances of delays or incomplete examinations. Here is what we recommend you bring to your appointment:

  • A complete list of all current medications, including dosages and prescribing physicians
  • Corrective lenses (glasses or contacts) if you wear them for driving
  • Hearing aids if applicable
  • Documentation from treating physicians for any ongoing medical conditions (diabetes, sleep apnea, cardiac history)
  • CPAP compliance report if you are treated for sleep apnea (minimum 4 hours per night, 70% of nights)
  • Any previous DOT medical examination forms or certificates

How often do I need a DOT physical?

Standard DOT medical certificates are valid for 24 months. However, if you have certain health conditions such as high blood pressure requiring medication, your examiner may issue a certificate valid for 12 months or less, requiring more frequent recertification.

What happens if I fail the DOT physical?

If you do not meet certification requirements, our examiner will explain exactly which criteria were not met and what steps you can take. In many cases, obtaining additional documentation from your treating physician or addressing a specific health metric can allow recertification at a follow-up visit.

Do I need an appointment for a DOT physical?

Is the DOT physical urinalysis a drug test?
